Artistry and Innovation for a Refreshed, Natural Look.

Age, genetics, and time can all cause eyelid skin to stretch and the muscles supporting them to weaken, resulting in a tired appearance. If you are struggling with heavy, tired-looking eyes or sagging skin around the eyes—especially severely sagging skin around the upper lids—eyelid surgery in New Jersey, clinically known as blepharoplasty and also known as an eye lift, is the definitive solution.

At New Jersey Plastic Surgery, we combine over 30 years of expertise with the latest surgical and non-surgical technologies. Our board-certified plastic surgeons offer unmatched expertise in restoring a bright, rested, and youthful appearance. Our focus is always patient-centric, whether you need a comprehensive solution for excess skin and fat or targeted treatment for bags under the eyes.

What is Eyelid Surgery (Blepharoplasty)?

Blepharoplasty is a type of eyelid surgery that is a precise procedure designed to lift and smooth the delicate skin, muscle, and fat around the upper and lower eyelids. Over time, the eyelids can develop excess skin, bulging fat pads, and pronounced lines that make you look perpetually fatigued, even when you are well-rested.

As pioneers in aesthetic medicine, our board-certified plastic surgeons, Dr. DiBernardo and Dr. Trovato, approach every blepharoplasty with meticulous attention to detail and an artful eye based in realism. This surgery removes or expertly repositions fat and tightens the surrounding skin, minimizing shadows and reducing puffiness. At NJPS, we use advanced tools and techniques to ensure maximum precision and patient comfort, delivering exceptional and natural-looking results that last.

Issues Addressed by Eyelid Surgery

The signs of aging around the eyes are complex. The combination of wrinkles, sagging, frown lines, and excess fatty tissue around the eyes can hinder your natural range of facial expressions. You may feel energetic and invigorated but only communicate exhaustion and dissatisfaction. With nearly 30 years of experience, board-certified plastic surgeon Dr. Barry DiBernardo has honed his surgical skill and established a reputation for quality and care.

He applies his years of experience with precision and an artful eye to remove excess skin, fat, and muscle from both the upper and lower eyelids. More than just the look of your eyelids, blepharoplasty can rejuvenate your facial appearance by addressing:

Heavy, Droopy Upper Lids and Bags

This is the most common issue, often resulting in upper eyelids that droop into your field of vision, which can contribute to a perpetually tired or sad appearance. In some cases, the tissue laxity is significant enough that it requires a brow lift in addition to the eyelid procedure to achieve optimal rejuvenation.

Vision Impairment

When excess eyelid skin on the upper lid partially blocks peripheral vision, especially the upper and outer parts, it is not just cosmetic—it is a medical concern. Our procedure can reduce this loss of peripheral vision, and the documentation we provide can often support an insurance claim.

Chronic Tired Appearance

Puffy or sagging lower lids can be due to herniated fat, creating prominent bags under the eyes. Addressing this requires either removing or repositioning the fat and tightening the skin to restore a consistently rested look.

Ptosis (Eyelid Droop)

In some cases, the problem is not lax skin but the underlying muscle that raises the lid, a condition called ptosis. While blepharoplasty targets skin and fat, true ptosis requires a specialized ptosis surgery to strengthen the muscle.

We specialize in advanced eyelid surgery that removes excess tissue and fat to correct these issues.

Types of Eyelid Surgery

Eyelid surgery isn't one-size-fits-all. Every eye is unique, which is why your treatment plan will be highly individualized. Dr. DiBernardo and Dr. Trovato will recommend the technique that best achieves your aesthetic goals:

Upper Eyelid Surgery (Upper Blepharoplasty)

The upper eyelid surgery procedure focuses on removing extra skin and fat from the upper eyelid. If your lids feel heavy or appear saggy, upper eyelid surgery can remove excess eyelid skin to open up your eyes. The incision is hidden in the natural crease. In cases where severely sagging skin around the brow line is the cause, a combination with a brow lift may be recommended. Lifting the upper lids not only helps you see more clearly but also makes your eyes look brighter and more open without changing the way you express yourself.

Lower Eyelid Surgery (Lower Blepharoplasty)

Lower blepharoplasty eyelid surgery is designed to smooth out puffiness, shadows, and bags under the eyes. Your surgeon will expertly redistribute excess fat muscle (or remove excess fat) and tighten the sagging skin. This is how a lower eyelid procedure delivers a more rested, natural appearance—like you finally got the sleep you've been missing.

Ethnic Blepharoplasty

With ethnic blepharoplasty, we take a thoughtful and respectful approach to enhancing the natural anatomy of the eyes, including creating or refining a natural-looking upper eyelid crease. This procedure is commonly requested by patients of East Asian descent, and it's important to note that it's not about westernizing the eyes—it's about enhancing your natural anatomy in a way that feels true to you. Our focus is always on creating balanced, authentic results that celebrate your unique features and goals.

Benefits of Eyelid Surgery

This isn't just about appearances. Yes, your eyes will look more awake and less puffy, but you might also notice you can see better, especially if extra skin is getting in the way of your upper vision. Here's what blepharoplasty can help with:

  • Tired-looking eyes, even when you feel well-rested - Lifting the eyelids can help you look more refreshed and energized, not worn out or sleepy.
  • Sagging skin that may be blocking part of your vision - Extra skin on the upper eyelids can sometimes hang over the lashes. This can be fixed to open up your visual field.
  • Puffiness or under-eye bags - Removing or repositioning fat in the lower lids smooths out that swollen or puffy look.
  • Loose skin that makes eye makeup harder to apply - With tighter skin around the eyes, makeup sits more evenly and doesn't smudge or collect in creases.
  • Confidence, especially in photos or social settings - When your eyes match how you feel on the inside, you may feel more comfortable in pictures and everyday interactions.

Am I a Candidate for Eyelid Surgery?

The best candidates for eyelid surgery are generally in good health and seek to eliminate droopy, baggy eyelids or vision issues caused by loose skin. During your initial consultation, we will assess the balance between your upper lid, lower lid, and brow, and determine if an upper blepharoplasty, lower blepharoplasty, or a combination procedure is right for you.

Ideal candidates typically:

  • Are in generally good health.
  • Do not have serious, active eye conditions (like severe dry eyes, glaucoma, allergies, circulatory problems, and thyroid issues).
  • Want to look more refreshed and eliminate the signs of eye aging.
  • Want to eliminate droopy, baggy eyelids.
  • Have hindered vision due to loose skin around the eyes.
  • Experience wrinkles or excess skin around the eyelids.
  • Are prepared to undergo plastic surgery and closely adhere to pre and postoperative instructions.

The Eyelid Surgery Procedure

Eyelid surgery is usually done right in our office or surgical suite. Depending on your specific plan, it can take anywhere from 45 minutes to a couple of hours. Most people are able to go home the same day. Before surgery starts, we'll make sure you're fully numb and relaxed. This might mean local anesthesia with some light sedation or full anesthesia, depending on what's best for you. Once you're comfortable, your surgeon makes tiny incisions in the natural creases of your eyelids so any marks are easy to hide as they heal.

Eyelid Surgery Results

You'll start to notice a difference in your eyes after the first couple of weeks, once most of the swelling and bruising settle down. The full results take a little longer, usually around three months, as the area continues to smooth out and fully heal. The change is subtle in the best way. You won't look like a different person, just more awake, refreshed, and maybe a little younger. And the results last. Most people enjoy the benefits of eyelid surgery for many years without needing any follow-up procedures. Good skincare and sun protection can help keep your eyes looking their best for even longer.

Eyelid Surgery Recovery

Dr. DiBernardo will provide you with detailed instructions for your eyelid surgery recovery. As an experienced board-certified plastic surgeon, Dr. DiBernardo has experience and skill in the area of facial rejuvenation. You can typically expect a quick and hassle-free recovery.

Your lids will tighten at first, but soon relax. For the day or two following your eyelid surgery, you may feel some minor discomfort at the incision sites. Your vision may be slightly blurred from swelling, but this quickly subsides. Using a cold compress and elevating your head for the first 24 hours alleviates some discomfort. Dr. DiBernardo will prescribe the necessary ointments and drops you will need. Wearing dark sunglasses can enhance comfort during this time. Stitches are usually removed in about a week after surgery and you can expect to return to work and normal activities within seven days. Blepharoplasty results typically last 10 to 20 years.

What About Eyelid Surgery Scars?

This is a common question and totally understandable. Any time there's an incision, there's a scar, but in this case, they're usually very hard to notice. The incisions are carefully placed in the natural creases of your eyelids, so once everything heals, the scars tend to blend in with the skin around them. Most people find that the scars become barely visible after a few months. If you follow your post-op instructions and take care of the area, there's a good chance they'll fade enough that no one will know you had surgery unless you tell them.

Eyelid Surgery (Blepharoplasty) FAQs

The rejuvenating effects of blepharoplasty are very long-lasting. For most patients, the results of upper eyelid surgery and lower eyelid surgery last between 10 to 20 years, often making it a one-time procedure. Good sun protection and skincare help preserve your results.

The procedure itself is not painful, as you will be under local or general anesthesia. Post-surgery, patients usually report a feeling of tightness or mild discomfort, rather than pain, which is easily managed with cold compresses and prescribed medication.

Yes, absolutely. Eyelid surgery is often combined with other procedures to achieve a comprehensive result. Common combinations include a Facelift or non-surgical treatments like laser resurfacing, injectables, or a TempSure lid lift. We will discuss the optimal combination for your unique goals during your consultation.

Not really. While lifestyle changes can help with overall puffiness, fat deposits in the eyelids usually don't go away without surgery.

There's no set age. Most people consider it in their 40s or 50s, but if sagging skin is affecting your vision or confidence earlier, it's okay to explore your options sooner.

If the sagging skin is getting in the way of your vision, insurance might help cover the upper eyelid surgery portion (functional blepharoplasty). If it's solely for appearance, it is considered cosmetic. We can provide documentation to help you determine your coverage during your consultation.

You'll want to sleep on your back with your head slightly elevated for the first week or two. This helps reduce swelling and keeps pressure off your eyes.
