(973) 509-2000. Our office staff will be pleased to schedule a consultation at our office at 29 Park Street in Montclair, NJ.
Our office is open from 9am to 5pm Monday, Tuesday and Thursday and from 8am to 4pm on Wednesday and Friday.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.
Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week. Call (973) 509-2000 and your doctor will be contacted immediately.
* Please Note: E-mail is not an emergency line. If you have an emergency you must call the answering service or the emergency room for a hospital in your area. All information that you provide via e-mail or through the use of data collection forms at this website will be included in your patient charts and records. Although e-mail and our website uses security precautions, please remember that the Internet is not secure.

If you have medical questions the medical staff can answer most routine questions. If you have medical questions that only a doctor can answer, a message will be taken and your medical file will be given to Dr. DiBernardo, who will return your call as soon as possible. If your question pertains to a non-medical matter, a qualified staff member would be pleased to assist you.
Our office does not participate with most health insurance plans. If you are eligible for out-of-network benefits, your insurance may cover certain reconstructive plastic surgery procedures. Covered procedures will vary with each insurance company; our office staff will assist you in determining your eligibility. Patients are responsible for deductibles, co-payments, and remaining balances. Patients also have the option of paying office and surgical fees in advance and then receiving their insurance reimbursement check directly from the insurance company.
For additional information please contact Brenda at (973) 509-2000.
Most medical insurance plans will not usually cover cosmetic procedures. Our office is able to assist you with a financing program for elective medical procedures. Patients may be pre-approved for financing with a low-interest monthly payment plan.
Brenda, our Patient Representative, would be pleased to provide you with further details. You may also check with your employer about medical services accounts for a tax-deductible payment of procedure.
We also accept the following credit cards: Visa, Mastercard, and Discover.
